Work Related Accidents

Claims MUST be submitted within 24 hours to be considered for payment. (Notify supervisor immediately after accident and take notice of any witnesses to the accident.) ALWAYS take care of any URGENT medical needs BEFORE filling out this form. (You can come to the Dining Services Office to fill out this form.) Please follow all instructions on the claim form. Print TWO copies and bring to the Dining Services Office.

New Employee Training

All employees are REQUIRED to have the following forms completed and turned into Dixie State Dining Services BEFORE their first day of employment.

 

To get your Food Handler’ Permit, the Southwest Utah Public Health Department offers the following class:

  • Once you complete the course please bring in a copy of your temporary permit that you can print from the website.
  • (When you receive your permanent card in the mail. Please bring in a copy of the front of the card to the Dining Office when you receive it.)
  • In-person: Please bring in a copy of the temporary permit that you will receive at the class.
  • (When you receive your permanent card in the mail. Please bring in a copy of the front of the card to the Dining Office when you receive it.)

You will receive your permanent card in the mail approximately 15 days after completing the course. If it has been longer than 20 days, please contact the Southwest Utah Health Department at 435-673-3528, or visit them in person at 620 South 400 East, St George, UT.

To receive your Paycheck, you must have Direct Deposit setup:

  • If you would like to have it deposited into your Checking Account, please bring a Voided Check.
  • If you would like to have it deposited into your Savings Account, please bring a Savings Deposit Slip

All employees need to review ALL pages of the Dining Services Employee Manual, and then print off the last page, sign and return.
